Effects on female depression treated with the combined therapy of acupuncture and the five-element music therapy

Objective To observe the clinical effects and safety on female depression treated with the combined therapy of acupuncture and the five-element music therapy. Methods A total of 120 female patients with depression were randomized into an electroacupuncture (EA) group, an acupuncture group, a five-element music therapy group and a group of the combined therapy of acupuncture and the five-element music therapy (combined therapy group), 30 cases in each one. In the EA group, the electric stimulation of acupuncture was applied at Baihui (GV 20) (GV 20), Shenting (GV 24) (GV 24), Neiguan (PC 6) (PC 6), Shenmen (HT 7) (HT 7), Sanyinjiao (SP 6) (SP 6) and Taichong (LR 3) (LR 3), with disperse–dense wave, 10 Hz/50 Hz, and 0.5 ms in wavelength. The needles were retained for 30 min. In the acupuncture group, the acupoints were same as the EA group, but without electric stimulation. In the five-element music therapy group, the music of Jiao tune was adopted in the treatment. In the combined therapy group, the acupuncture treatment and the five-element music therapy were the same as the acupuncture group and the five-element music therapy group, in which the music therapy was followed after acupuncture treatment. The treatment was given once a day, at 2 days of interval after every 5 treatments in a week, totally for 8 weeks. The Hamilton depression scale (HAMD), Hamilton anxiety scale (HAMA) before and after treatment and the treatment emergent symptom scale (TESS) after treatment were observed in each group. The clinical effects and safety were compared among the groups. Results The total effective rate was 86.7% (26/30) in the combined therapy group, better than 66.7% (20/30) in the EA group, 63.3% (19/30) in the acupuncture group and 53.3% (16/30) in the five-element music therapy group ( P < 0.01, P < 0.05). After treatment, HAMD and HAMA scores were all reduced obviously in the patients of the 4 groups (all P < 0.01), and the scores in the combined therapy group were lower than the other 3 groups (all P < 0.01). The TESS score in the combined therapy group was lower than the other 3 groups (all P < 0.01) and the score in the five-element music therapy group was lower than the EA group and the acupuncture group (both P < 0.01). Conclusion In the female patients, the combined treatment with acupuncture and the five-element music therapy obviously reduces the severity of depression and anxiety, as well as the adverse reactions. The effects are better than the simple application of EA, acupuncture or music therapy.
Effects on polycystic ovary syndrome of abdominal obesity treated with the dialing needling method at the abdomen

Objective To observe the clinical effects on polycystic ovary syndrome (PCOS) of abdominal obesity treated with the dialing needling technique at the abdomen. Methods A total of 33 patients of PCOS of abdominal obesity were treated with the dialing needling method. The reinforcing technique was applied to Zhongwan (CV 12), Qihai (CV 6), Guanyuan (CV 4) and Zhongji (CV 3) and the reducing technique was applied to Huaroumen (ST 24), Tianshu (ST 25), Shuifen (CV 9), Luanchaoxue (Extra), Huangshu (KI 16) and Daimai (GB 26). The electric stimulation was added at Huaroumen (ST 24), Tianshu (ST 25) and bilateral Daimai (GB 26). The treatment was given once every two days, three times a week. The treatment was discontinued during the menstruation, 10 treatments made one course. Totally, six courses of treatment were provided. The body mass index (BMI) and the ovarian volume were observed before and after treatment. The levels of serum fasting insulin (FINS), fasting blood glucose (FBG), serum follicle stimulating hormone (FSH), luteinizing hormone (LH), testosterone (T) and prolactin (PRL) were determined before and after treatment. Results As compared with those before treatment, BMI was reduced obviously ( P < 0.01), the ovarian volume was shrunk obviously ( P < 0.01) and the levels of serum FINS, LH and T were reduced obviously ( P < 0.05, P < 0.01) after treatment. The differences in serum PRL and FSH were not significant before and after treatment (both P > 0.05). Conclusion The dialing needling method reduces the body mass and ovarian volume in the patients of PCOS of abdominal obesity type. The effect mechanism is relevant with the reducing of FNS, LH and T.

Objective To observe the differences in the sleep quality and the effects on insomnia severity between He’s three-promotion needling method and the oral administration of estazolam. Methods A total of 70 patients of subacute insomnia were randomized into a three-promotion needling group and a medication group, 35 cases in each one. In the three-promotion needling group, the mild promotion technique with the filiform needle, the warm promotion technique with the fire needle and the strong promotion technique with the three-edged needle were adopted in combination. The acupuncture with the filiform needle was used at Baihui (GV 20), Sishencong (EX-HN1), Shenting (GV 24), Shenmen (HT 7), etc. The treatment was given three times a week, once every two days. The pricking technique with fire needle was applied to Ganshu (BL 18), Xinshu (BL 15), Feishu (BL 13), Pishu (BL 20), Shenshu (BL 23) and Geshu (BL 17), once every three days, twice a week. The bleeding technique with the three-edged needle was used at the ear apex, once every two days, three times a week. In the medication group, the estazolam tablets were taken orally before sleep, 1mg once every two days. The treatment was given consecutively for four weeks in the two groups. Separately, before treatment, in 2-week treatment and in 4-week treatment as well as in 1-month follow-up after treatment, the Pittsburgh sleep quality index (PSQI) and the insomnia severity index (ISI) scores were recorded in the patients of the two groups. The clinical effects were evaluated in the two groups. Results In 2 weeks and 4 weeks of treatment, as well as in the follow-up, PSQI scores and ISI scores were all reduced obviously as compared with those before treatment in the two groups (all P < 0.05). In the three-promotion needling group, the PSQI score at each time point was reduced much more obviously as compared with the medication group (all P < 0.05). In the follow-up, the ISI score in the three-promotion needling group was lower than the medication group ( P < 0.05). The total effective rates were 97.2% (34/35) and 91.4% (32/35) at the end of treatment and in the follow-up respectively in the three-promotion needling group, higher than 85.7% (30/35) and 71.4% (25/35) in the medication group (both P < 0.05). Conclusion He’s three-promotion needling method much more effectively improves the sleep quality and reduces insomnia severity in the patients of subacute insomnia. This therapy presents the satisfactory short-term and long-term effects.
Mechanism of “ Xingnao Kaiqiao” acupuncture for the opening of ATP sensitive potassium channel against cerebral ischemia reperfusion injury in rats

Objective To observe the mechanism of “ Xingnao Kaiqiao” acupuncture for the opening of ATP sensitive potassium channel (K ATP) against cerebral ischemia reperfusion injury in rats. Methods Eighty-four rats were randomly divided into a sham-operation group, a model group, an electroacupuncture (EA) group, an EA + K ATP blocker group, 21 rats in each group. Totally 10 μL intracerebral injection with glipizide (1 μmol/5μL) was used in the EA + K ATP blocker group. The cerebral ischemia reperfusion model was established by Zea Longa’s suture method in the model group, the EA group and the EA + K ATP blocker group. Rats in the sham-operation group were received the same surgery but without nylon filament insertion. Acupuncture (20 min a time) was performed at “Neiguan” (PC 6), “Shuigou” (GV 26) and “Sanyinjiao” (SP 6) in the EA group and the EA + K ATP blocker group at 10: 00 and 16: 00 for three days, firstly 90 min after model establishment. EA (2 Hz/15 Hz, 1 mA) was connected at the affected “Neiguan” (PC 6) and “Sanyinjiao” (SP 6). The same fixation was used in the sham-operation group and the model group, without EA. Neurological function was assessed by Zausinger’s neurologic assessment scale. 2,3,5-triphenyltetrazolium chloride (TTC) staining was used to detect infarct volume. Neurocyte apoptosis in the hippocampus was detected by flow cytometry and the protein expressions of B lymphocytoma-2 gene (Bcl-2) and B cell lymphoma factor-associated X protein (Bax) were measured by Western-blot. Results In comparison with the model group, the neurological score of the EA group increased ( P < 0.01); the infarct volume and the hippocampal neuron’s total apoptosis rate of the EA group decreased (both P < 0.05); the protein expression of Bcl-2 and Bcl-2/Bax of the EA group increased ( P < 0.05, P < 0.01); and the protein expression of Bax of the EA group decreased ( P < 0.01). Compared with the EA group, the neurological score of the EA + K ATP blocker group decreased ( P < 0.05); the total apoptosis rate of hippocampus neurons of the EA + K ATP blocker group increased ( P < 0.05); the expression of Bcl-2 protein of the EA + K ATP blocker group reduced ( P < 0.05); the expression of Bax protein of the EA+K ATP blocker group increased ( P < 0.05). Conclusion “ Xingnao Kaiqiao” acupuncture has brain protective effect on rats with focal cerebral ischemia reperfusion injury. The mechanism may be related to regulating the opening of K ATP channels and decreasing the apoptosis of neurons.

Effect of electroacupuncture on denervated skeletal muscle atrophy and expression of IGF-1/PI3K/AKT in rats

Objective To observe the effects of electroacupuncture (EA) on denervated skeletal muscle atrophy and expression of IGF-1/PI3K/AKT signaling pathway in rats, and to explore the possible mechanisms of it. Methods Thirty-six male SD rats were randomly divided into a control group, a model group and an EA group. Denervated muscle atrophy model was made by cutting off the left hindlimb sciatic nerve in the model group and the EA group. In the EA group, EA, dense-disperse wave, 2 Hz/33 Hz and 1.5–2 mA, was connected at Zusanli (ST 36) and Shangjuxu (ST 37). The treatment was given for 30 min every time, once a day and a total of 20 times were needed, there was 2 days interval every 5 times. The rats in the control group and model group were fixed in the same cage every day but not applied EA. Sciatic nerve function index (SFI) and ratio of muscle wet weight (RWW) were measured after treatment. HE staining was used to determine the ratio of cross-sectional area (RCA) and the ratio of fiber diameter (RFD). TUNEL and double immunofluorescence staining were used to evaluate the myocyte apoptosis, proliferation and differentiation. The mRNA expressions of IGF-1 and AKT were detected by real-time PCR. The total protein of AKT, p-AKT (Ser473) and p-AKT (Thr308) were detected by western blot. Results Compared with the control group, SFI, RWW, RCA and RFD of the model group were significantly decreased and apoptotic index of myocytes were significantly increased (all P < 0.01). Compared with the model group, the above indexes of the EA group were improved (all P < 0.05), and the expressions of PAX3 and PAX7 related to proliferation and differentiation of muscle satellite cells were significantly increased (both P < 0.01), p-AKT (Ser473) protein level was increased ( P < 0.05). Conclusion EA can improve the atrophy of denervated muscles, reduce myocyte apoptosis, and promote the proliferation and differentiation of muscle satellite cells. The mechanism may be related to the increase of p-AKT (Ser473) level and activation of AKT signaling pathway.
Correlation of the electric excitability treated with electroacupuncture at different acupoints and the prognosis of Bell’s palsy

Objective To explore the correlation of the electric excitability treated with electroacupuncture (EA) at different acupoints and the prognosis of Bell’s palsy, using the Logistic regression analysis. Methods A total of 93 patients of Bell’s palsy were collected in the retrospective analysis. Acupuncture was applied to Cuanzu (BL 2), Yangbai (GB 14), Yingxiang (LI 20), Dicang (ST 4), Xiaguan (ST 7) and Qianzheng (Extra point) on the affected side. The electric stimulation was added at the three pairs of points, named Cuanzu (BL 2) and Yangbai (GB 14) (A group), Yingxiang (LI 20) and Dicang (ST 4) (B group) and Qianzheng (Extra point) and Xiaguan (ST 7) (C group), continue wave, 1 Hz at current frequency, for 30 min in one treatment. EA was given once every two days. The endpoint outcome evaluation was conducted when the facial nerve function was recovered completely after treatment or in 16 weeks after onset. On the 8th day of onset, the prognosis-related indexes were collected, such as the electric excitability grade in the A, B and C groups, gender, age, the facial nerve lesion segment and type 2 diabetes mellitus (T2DM). The Logistic regression analysis was conducted on these indexes and the prognosis. Results The single factor Logistic regression analysis showed that the gender, age and the electric excitability of the C group were negatively correlated with the prognosis statistically ( P > 0.05). T2DM, the facial nerve lesion segment and the electric excitability of the A and B groups were positively correlated with the prognosis statistically ( P < 0.05). The multivariate Logistic regression analysis indicated that the electric excitability in the A and B groups was the independent predictor of Bell’s palsy prognosis ( P < 0.05). ORvalues were 8.410 (1.176–60.113) and 12.589 (1.355–116.922) respectively. Conclusion The electric excitability of EA at the pairs of Cuanzu (BL 2) and Yangbai (GB 14), and of Yingxiang (LI 20) and Dicang (ST 4) is one of the indexes for the prognosis evaluation of Bell’s palsy. It contributes to the early identification of the severity of facial nerve lesion, the regulation of the therapeutic regimen and the improvement of the clinical effects.

Objective To observe the change of the specificity of the microcirculatory blood perfusion at the area of “Feishu” (BL 13) in the rats of chronic obstructive pulmonary disease (COPD). Methods According to the random number table, 60 Wistar rats were divided into a 29 d model No. 1 group (C1 group), a 29 d normal control No. 1 group (N1group), a 89 d model No. 2 group (C2 group) and a 89 d normal control No. 2 group (N2 group), with 15 rats in each one. In the C1 and C2 groups, the smoking and intratracheal dripping of endotoxin were used in combination to prepare COPD model. The rats were fed normally in the N1 and N2 groups. “Feishu” (BL 13), “Xinshu” (BL 15), the control point lateral to “Feishu” (BL13) and the control point lateral to “Xinshu” (BL 15) were selected as the monitoring points. The pericam perfusion speckle imager (PeriCam PSI System) was adopted to monitor the microcirculatory blood perfusion unit (PU) at the monitoring points before and after 29 d and 89 d of modeling separately. Results Before modeling, the differences in PU were not significant at each monitoring point in comparison among the four groups and the differences were not significant among “Feishu” (BL 13) and “Xinshu” (BL 15) as well as their lateral sites (all P > 0.05). After modeling, PU was increased at each monitoring point in the C1 and C2 groups (all P < 0.05). PU in the C1 group was higher than the N1 group and PU in the C2 group was lower than that in the N2 group. PU at each monitoring point in the C1 group was higher than that in the C2 group, with significant differences (all P < 0.05). In the C1 and C2 groups, the specific change occurred. PU at “Feishu” (BL 13) was higher than its lateral site. But such specific change did not happen in the N1 and N2 groups. Conclusion PU at “Feishu” (BL 13) presents the specific change relevant with the sickness duration in the COPD rats.
Clinical study of electroacupuncture on perioperative analgesia in patients with thyroid surgery under cervical plexus block

Objective To investigate the change trend and correlation between the perioperative pain degree and neuroendocrine hormone levels in the perioperative period on patients with thyroid surgery by EA combined with cervical plexus block. Methods Forty patients with elective thyroidectomy were enrolled and divided into a control group and an observation group according to the random number table method, 20 cases in each group. In the control group, cervical plexus block was performed. In the observation group, the cervical plexus block combined with EA (EA) was given. After the cervical plexus block was completed, the EA was applied at the bilateral Hegu (LI 4) (LI 4) and Neiguan (PC 6) (PC 6) for 30 min. The visual analog scale (VAS) was used to assess the pain level before anesthesia (T 0), 2 h after operation (T 1), 12 h after operation (T 2), 24 h after operation (T 3) and 48 h after operation (T 4). Blood samples such as adrenocorticotropic hormone (ACTH), C-reactive protein (CRP) and β-endorphin (β-EP) were collected to measure the level of neuroendocrine hormones, and to analyze the correlation between ACTH, CRP, β-EP and VAS scores. Results The VAS scores at T 1, T 2, T 3 were higher than those at T 0 in the two groups (all P < 0.05). The VAS scores T 1, T 2 and T 3 in the observation group were lower than those in the control group (all P < 0.05). The ACTH and CRP in the control group at T 1, T 2and T 3 were significantly higher than those at T 0 (all P < 0.05). The β-EP in the observation group at T 1, T 2and T 3 was significantly higher than that at T 0 (all P < 0.05). The ACTH and CRP in the observation group at T 1, T 2 and T 3 were lower than those in the control group, and the β-EP level was higher than that in the control group (all P < 0.05). The VAS score was significantly positively correlated with ACTH and CRP levels, and negatively correlated with β-EP. Conclusion EA stimulation can inhibit the secretion of ACTH and CRP, promote the release of β-EP, reduce the perioperative pain score, and inhibit the body’s stress response.
